Dr. Chacko’s journey into medicine was not accidental; it was shaped by the realities of the world he grew up in. Born in Nigeria to Malayali parents, he moved to South Africa at the age of five, spending his formative years in the Eastern Cape. His parents were teachers at rural schools, and through their eyes, he saw the stark challenges faced by pupils and their families living in underserved areas. These early lessons in empathy and social reality would later form the bedrock of his medical philosophy. 

While studying medicine at Wits, He found his specific calling in a field that often operates behind the scenes but wields immense influence: radiology. “I discovered radiology and immediately recognised its power,” he recalls. “Imaging sits quietly at the centre of clinical decision making. It is precise, decisive, and deeply impactful.” He was captivated by the ability to alter the entire trajectory of a patient’s life with a single, clear image. This realization led him into diagnostic radiology and eventually to the University of Bristol, where he completed his PhD, further honing his capacity for deep analysis. 

The 70-Kilometre Gap 

For many doctors, the pinnacle of success is a thriving private practice, known for technical excellence and comfort. Dr. Anith achieved this, but he found that technical proficiency wasn’t enough to satisfy his deeper sense of purpose. In private practice, he began to notice a heartbreaking pattern. He met countless women who were presenting with advanced stages of breast cancer—diagnoses that came too late for simple interventions. 

The tragedy wasn’t just the disease; it was the logistics. “The barriers were predictable and systemic: distance, cost, transport, time, and lack of awareness,” he explains. He realized that many of these women lived less than 70 kilometers from a mammography center, yet that short distance was an insurmountable chasm due to a lack of transport or funds. “These moments crystallised something I had been feeling for years: the healthcare system was waiting for patients who had no realistic way of reaching it.” 

It became clear to him that the traditional model of fixed-site radiology could never scale fast enough to meet the needs of South Africa’s rural and peri-urban populations. If the patients couldn’t come to the clinic, the clinic had to go to them. “I wanted to build the opposite,” he says. “A service that went to them.” 

Building Radhiant: A Mission on Wheels 

Leaving the stability of private practice to launch a mobile imaging company was a massive career shift involving significant risk and sacrifice. Yet, for Dr. Chacko, it was the only logical step to align his career with his purpose. He founded Radhiant Diagnostic Imaging with a bold vision: to offer high-quality, accessible screening to communities that had been left off the healthcare map. 

Today, as the Founder and CEO of the Radhiant Group, which includes RDISA and Radhiant Technologies, He wears many hats. His role is a complex blend of strategy, operational oversight, and financial structuring. However, he refuses to become a distant executive. “I still personally oversee the reporting of all mammograms done by RDISA to stay connected to the frontline mission,” he notes. This discipline ensures that he never loses sight of the individual patients behind the statistics. 

His daily focus is on building the systems and culture necessary to scale responsibly. He understands that passion alone cannot drive a nationwide healthcare logistics operation; it requires rigorous discipline and forward-looking leadership. 

Engineering Against the Elements 

Taking sensitive, high-tech imaging equipment onto the roads of South Africa is not a task for the faint-hearted. He and his team faced a litany of infrastructure challenges that would daunt most logistical planners. “South Africa’s geography and infrastructure present real challenges,” he admits. “Terrain, long travel distances, weather conditions, electrical instability, and safety issues all had to be engineered around.” 

To overcome these hurdles, Radhiant couldn’t just rely on standard solutions. They had to innovate. Operationally, this meant establishing standardized protocols and relying on highly trained staff. Trust was another major hurdle; they had to build relationships in communities that were unfamiliar with mobile diagnostics. “We overcame these challenges through disciplined systems, rigorous training, strong local engagement, and continuous innovation in how our units are designed and operated,” Dr. Anith explains. 

A common misconception about mobile healthcare is that mobility comes at the cost of quality. He is quick to dismantle this idea. “Quality is foundational,” he asserts. Each mobile unit is a marvel of engineering, equipped with hospital-grade stabilization and redundancy systems. The team adheres to strict daily calibration checks and real-time performance monitoring to ensure that a mammogram taken in a remote village is of the exact same diagnostic quality as one taken in a city hospital.

Dignity in Diagnostics: The Makhanda Initiative 

The impact of this rigorous approach is best illustrated by the company’s initiative in Makhanda in 2023. This project highlighted exactly why mobile imaging is more than just a medical service—it is a restoration of dignity. For many of the women screened during this initiative, it was the first time they had ever undergone a mammogram. 

The clinical results were immediate and vital: several early-stage cancers were detected, allowing for timely treatment that saved lives. But for Dr. Chacko, the emotional impact was equally profound. “What stood out most was the sense of dignity expressed by women who said they felt seen for the first time,” he shares. “In communities that have never had access, mobile mammography turns healthcare from an abstract service into a lived experience of inclusion.” 

Since that initiative, Radhiant has successfully replicated this model across the country, introducing mobile mammography to over 400 locations so far. This expansion proves that the model is not just a pilot project, but a sustainable solution for the nation. 

Measuring Success and Looking to 2028 

Dr. Anith measures the success of Radhiant not just by financial metrics, but by reach and clinical outcomes. The numbers speak for themselves. In just its first three years, starting with a single truck in 2023, the organization has screened more than 18,000 women across South Africa. More importantly, this has led to the early detection of over 370 breast cancers—hundreds of lives potentially saved because the barrier of distance was removed. 

However, He is not one to rest on past achievements. His vision for the future is aggressive and deeply optimistic. “Our primary goal is to reach 500,000 women annually by 2028,” he declares. To achieve this tenfold increase in impact, Radhiant plans to scale from its current fleet of seven mobile units to more than sixty. 

The roadmap includes deepening partnerships with corporates and insurers, expanding operations into the wider Southern African Development Community (SADC) region, and integrating Artificial Intelligence into their diagnostic workflow. “The broader mission is to prove that high quality imaging can be delivered at an even greater scale… without compromise, and especially to communities that have traditionally been left out,” he affirms.

The Evolution of a Leader 

Leading such a complex, high-growth organization requires a skillset that goes far beyond reading X-rays. He credits a combination of his diverse experiences for his leadership capabilities. His specialist medical training taught him precision; his PhD developed his problem-solving skills; and his time in private practice exposed him to the realities of business. 

But to lead Radhiant, he had to grow even further. He immersed himself in leadership development programs like Strategic Coach, Scaling Up, and Higher Ground Leadership, alongside using assessment tools like Kolbe and Lumina. “These helped me understand myself, how to build teams, systems, and culture capable of scaling at speed,” he says. He operates on the belief that for the company to grow fast, he must grow as a leader just as quickly. 

To maintain this high level of performance, He prioritizes recharging. He finds balance through travel, reading, and fitness, as well as exploring ideas in behavioral science and technology. Most importantly, he stays grounded through time with his family. “My most rejuvenating activity though, is spending quality and unstructured time with my 9-year-old daughter who helps me keep things in perspective and to live in the moment,” he says warmly. 

The Future of Imaging in Africa 

Looking ahead to 2025 and beyond, Dr. Anith is excited about the convergence of technology and access. He sees AI-assisted mammography and predictive analytics as key tools to standardize quality and support overstretched radiology systems. He is particularly enthusiastic about cloud-based workflows that can integrate mobile imaging with population health data. “This combination allows us to move from reactive care to proactive screening at scale and to influence national policy,” he predicts. “The future is decentralised, intelligent, and radically accessible.” 

Ultimately, Dr. Anith hopes his work serves as a beacon for other healthcare professionals across the continent. His message is one of empowerment. “I hope our work shows that innovation does not always require new technology. Sometimes it requires courage to challenge entrenched systems and a willingness to build for those who have been overlooked,” he concludes.
